Cardboard Type, Says: \"Mapledale Dairy Chocolate Flavored Milk\" on the Lid. It also says: \"Pasteurized\". \"Contains: Sugars, cocoa chocolate, malt extract, carrageen extract, salt, natural and artificial flavors, whole milk\". \"Phillipsburg, N.J. R.D. #2\".

The disc is approximately1 11/16 inch diameter. The top of the lid has a small area where you can put your finger nail to pull up the lid to get it off the milk bottle.This disc would fit on a milk bottle that has a small ledge just inside of the top of the bottle.
